Emergency Diaper Assistance

This form should only be completed by COMMUNITY PARTNERS, ORGANIZATIONS, OR SOCIAL SERVICE AGENCIES REFERRING A CLIENT(S) FOR EMERGENCY DIAPER ASSISTANCE. 

The family needing Emergency Assistance should meet at least one of the following:
~Recent homelessness
~Recent domestic violence
~Disability of the mother or child
~Kinship Foster Care
~Be a crisis case based on the information provided by the referring agency

The prospective client must also:
~Be on WIC or SNAP and be able to present an active card
~Have a valid photo ID

Families can receive up to 100 diapers, wipes, and other baby essentials based on the circumstances of their case.

Donation pickup is at our office location (2711 S Ervay #102, Dallas, Texas 75215) unless otherwise coordinated with the pantry.
